Where can I get my fingerprints and background check?
Fingerprinting and background checks are typically available at county sheriff’s offices, local police departments, or approved third-party providers in Louisiana.


Where can I obtain photos for licensing applications?

Most pharmacies (such as CVS or Walgreens) offer passport photo services, as do local Department of Motor Vehicle (DMV) offices and some tag agencies.


Required Documentation for New Licensing Applicants

US Citizens:

  • Certificate of Completion from an Approved Examination Entity
  • Affidavit of Lawful Presence (signed)
  • A valid, unexpired Driver’s License OR State Issued Photo ID OR Military ID AND
  • Birth Certificate OR Social Security Card OR valid, unexpired Passport OR W-2/1099 form from current employer
  • Two completed fingerprint cards


Non-US Citizens:

  • Verification of Immigration Status
  • Certificate of Completion from an Approved Examination Entity
  • Affidavit of Lawful Presence (signed)
  • A valid, unexpired Driver’s License OR State Issued Photo ID OR Military ID AND
  • Birth Certificate OR Social Security Card OR valid, unexpired Passport OR W-2/1099 form from current employer
  • Two completed fingerprint cards


Note: All new applicants must appear in person at the Office of the State Fire Marshal office or an approved facility.


Exceptions
A Certificate of Completion from an Approved Examination Entity is not required if applying only for an exempt category (if applicable).
